Information Collection And Use
While using our Site, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you. Personally identifiable information may include, but is not limited to, your name ("Personal Information"), email address, phone number, and geographic location. If you enroll in coaching or webinar services, we may also collect health-related information relevant to your tobacco cessation goals ("Health Information"). Your information will never be sold or transferred to any other website or organization for any reason.

Use of Health Information
Health Information you share with us is used solely to provide coaching and related cessation services. We do not use Health Information for any purpose other than delivering the service you requested. Frank Oden is a tobacco cessation coach, not a licensed medical or mental health provider, and this practice is not a HIPAA Covered Entity. We nonetheless apply safeguards consistent with HIPAA's confidentiality principles to protect Health Information.

Confidentiality and Security
Frank Oden is the only person with access to client Health Information. Session notes do not contain personal identification information. We do not process or store payment information; payments are handled directly by Square, and business and financial records are maintained by Square, not on our own systems. The only personal information we retain is basic contact and intake information you provide. While we take reasonable steps to protect your information, no method of transmission over the Internet, or method of electronic storage, is 100% secure, and we cannot guarantee its absolute security.

Log Data
Like many site operators, we collect information that your browser sends whenever you visit our Site ("Log Data"). This Log Data may include information such as your computer's Internet Protocol ("IP") address, browser type, browser version, the pages of our Site that you visit, the time and date of your visit, the time spent on those pages and other statistics such as your general geographic location. We may use your Personal Information to contact you with newsletters, marketing or promotional materials and other information related solely to Denver Tobacco Clinic news, offers and activities.
